Sarcopenia em cirurgia reconstrutiva ortopédica: considerações gerais
Introdução: A sarcopenia é definida como uma síndroma caracterizada pela perda progressiva e generalizada de massa muscular esquelética, e da sua função (perda de força e/ou deterioração do desempenho físico), associada ao envelhecimento, que leva a um risco aumentado de resultados menos conseguidos, nomeadamente, limitação funcional, baixa qualidade de vida e morte precoce. A sarcopenia é um termo recente no glossário médico, e pouco se sabe, em concreto, sobre esta síndroma geriátrica, não só na área de Ortopedia, mas também nas outras especialidades cirúrgicas e médicas. Seja como for, sabe-se que os doentes sarcopénicos são mais propensos a quedas, fraturas e a uma limitação da mobilidade, o que torna a sarcopenia num tema de estudo prioritário na especialidade de Ortopedia.
Estima-se que 44% dos doentes que são submetidos a cirurgia ortopédica, padeçam desta condição, com um risco acrescido de complicações pós-cirúrgicas e na reabilitação funcional. Assim, este trabalho de revisão bibliográfica tem como objectivo, abordar a prevalência e as repercussões da sarcopenia nos idosos com lesões ortopédicas, nomeadamente fraturas incidindo, também, na mortalidade associada à sarcopenia, no diagnóstico, prevenção e seu tratamento.
Materiais: Recorreu-se a uma pesquisa de artigos científicos e de revisão, relacionados com o tema "sarcopenia in orthopedic surgery", na base de dados “PubMed”, entre Junho de 2016 e Fevereiro de 2017. Os termos de pesquisa pré-definidos usados foram, entre outros, ‘sarcopenia’, ‘orthopedics’, ‘surgery’, ‘diagnosis’, ‘prevalence’, ‘complication’, ‘intervention’, ‘treatment’, ‘mortality’, ‘nutrition’ e ‘exercise’.
Resultados: Apesar de ainda não se encontrarem completamente aceites os critérios e métodos de diagnóstico da sarcopenia, vários estudos têm sido publicados, seguindo as recomendações da EWGSOP. Estas recomendações especificam que 2 de 3 critérios – baixa massa muscular, diminuição da força e do desempenho físico – devem estar presentes para diagnosticar um individuo com sarcopenia, em que um deles, a massa muscular, deve estar obrigatoriamente presente. Também categorizaram a sarcopenia em 3 estádios, pré-sarcopenia, sarcopenia e sarcopenia severa. Vários estudos comprovam a associação da sarcopenia com a mortalidade (de causas não especificadas) dos doentes que apresentam esta síndroma, havendo maior taxa de mortalidade nos sarcopénicos, comparativamente a grupos de controlo. A osteoporose, apesar de já se saber ser uma das causas de fraturas de osso frágil, também foi demonstrado que está associada à sarcopenia, sendo que, diversos estudos confirmam que os sarcopénicos apresentam um aumento do risco de padecer de osteoporose, e que a própria sarcopenia é um factor de risco independente para fraturas de osso frágil. Por outro lado, alguns estudos demonstram que há uma alta prevalência de sarcopenia em doentes com fraturas da anca e fraturas vertebrais. O tratamento/prevenção da sarcopenia, ainda não foi tema de estudos relevantes, não havendo ainda um consenso sobre a nutrição e exercícios a prescrever, apesar de alguns trabalhos mostrarem bons resultados quando entrosaram as alterações dietéticas e o exercício físico.
Discussão: Verificou-se que existe uma forte associação entre sarcopenia e mortalidade, entre sarcopenia e osteoporose, e que a sarcopenia aumenta o risco de fraturas de osso frágil, independentemente da presença concomitante de osteoporose ou não. Também foi demonstrado que a sarcopenia é muito prevalente em doentes com fraturas da anca, das vértebras e distais do rádio. Um artigo confirmou que a taxa de complicações e do tempo de internamento foi superior em doentes sarcopénicos após cirurgia da coluna toracolombar, para além de não atingir a mesma capacidade funcional que os doentes não sarcopénicos.
Conclusão: Torna-se imprescindível uma maior investigação sobre as formas de prevenir e/ou tratar a sarcopenia. Em primeiro lugar, é necessário definir os critérios e métodos de diagnóstico, assim como conhecer as complicações pós-operatórias da cirurgia ortopédica em doentes sarcopénicos, matéria insuficientemente estudada. Por outro lado, é preciso perceber as repercussões da sarcopenia na cirurgia ortopédica, uma vez que os doentes sarcopénicos apresentam patologias diversas do foro ortopédico.info:eu-repo/semantics/publishedVersio
Galaxy Morphological Segregation in Clusters: Local vs. Global Conditions
We study the relative fraction of galaxy morphological types in clusters, as
a function of the projected local galaxy density and different global
parameters: cluster projected gas density, cluster projected total mass density
, and reduced clustercentric distance. Since local and global densities are
correlated, we have considered different tests to search for the parameters to
which segregation show the strongest dependence. Also, we have explored the
results of our analysis applied to the central regions of the clusters and
their outskirts. We consider a sample of clusters of galaxies with temperature
estimates to derive the projected mass density profile and the 500 density
contrast radius () using the NFW model and the scaling relation
respectively. The X-ray surface brightness profiles are used to obtain the
projected gas density assuming the hydrostatic equilibrium model. Our results
suggest that the morphological segregation in clusters is controlled by the
local galaxy density in the outskirts. On the other hand, the global projected
mass density, shows the strongest correlation with the fraction of
morphological types in the central high density region, with a marginal

Violation of the Ikeda sum rule and the self-consistency in the renormalized quasiparticle random phase approximation and the nuclear double-beta decay
The effect of the inclusion of ground state correlations into the QRPA
equation of motion for the two-neutrino double beta () decay
is carefully analyzed. The resulting model, called renormalized QRPA (RQRPA),
does not collapse near the physical value of the nuclear force strength in the
particle-particle channel, as happens with the ordinary QRPA. Still, the
transition amplitude is only slightly less sensitive on
this parameter in the RQRPA than that in the plain QRPA. It is argued that this
fact reveals once more that the characteristic behaviour of the
transition amplitude within the QRPA is not an artifact of
the model, but a consequence of the partial restoration of the spin-isospin
symmetry. It is shown that the price paid for bypassing the collapse in
the RQRPA is the violation of the Ikeda sum rule.Comment: 16 pages, latex, 3 postscript figure
On the energy-shell contributions of the three-particle~-~ three-hole excitations
The response functions for the extended second and third random phase
approximation are compared. A second order perturbation calculation shows that
the first-order amplitude for the direct excitation from the ground
state cancels with those that are engendered by the - coupling. As
a consequence nonvanishing effects to the response involve off

The harmonic oscillator on Riemannian and Lorentzian configuration spaces of constant curvature
The harmonic oscillator as a distinguished dynamical system can be defined
not only on the Euclidean plane but also on the sphere and on the hyperbolic
plane, and more generally on any configuration space with constant curvature
and with a metric of any signature, either Riemannian (definite positive) or
Lorentzian (indefinite). In this paper we study the main properties of these
`curved' harmonic oscillators simultaneously on any such configuration space,
using a Cayley-Klein (CK) type approach, with two free parameters \ki, \kii
which altogether correspond to the possible values for curvature and signature
type: the generic Riemannian and Lorentzian spaces of constant curvature
(sphere , hyperbolic plane , AntiDeSitter sphere {\bf
AdS}^{\unomasuno} and DeSitter sphere {\bf dS}^{\unomasuno}) appear in this
family, with the Euclidean and Minkowski spaces as flat limits.
We solve the equations of motion for the `curved' harmonic oscillator and
obtain explicit expressions for the orbits by using three different methods:
first by direct integration, second by obtaining the general CK version of the
Binet's equation and third, as a consequence of its superintegrable character.
The orbits are conics with centre at the potential origin in any CK space,
thereby extending this well known Euclidean property to any constant curvature
configuration space. The final part of the article, that has a more geometric
character, presents those results of the theory of conics on spaces of constant
curvature which are pertinent.Comment: 29 pages, 6 figure
Neutrino-Nucleus Reactions and Muon Capture in 12C
The neutrino-nucleus cross section and the muon capture rate are discussed
within a simple formalism which facilitates the nuclear structure calculations.
The corresponding formulae only depend on four types of nuclear matrix
elements, which are currently used in the nuclear beta decay. We have also
considered the non-locality effects arising from the velocity-dependent terms
in the hadronic current. We show that for both observables in 12C the higher
order relativistic corrections are of the order of ~5 only, and therefore do
not play a significant role. As nuclear model framework we use the projected
QRPA (PQRPA) and show that the number projection plays a crucial role in
removing the degeneracy between the proton-neutron two quasiparticle states at
the level of the mean field. Comparison is done with both the experimental data
and the previous shell model calculations. Possible consequences of the present
study on the determination of the neutrino oscillation
